Important Information for Your Scalp Micropigmentation (SMP) TreatmentThis information supplements what you've already received (Client Personal Medical Record, consultation info, and our website). It helps you make an informed decision about SMP treatment. Please read this, prior documents, and consultation notes carefully. Sign the attached Personal Medical Record only if you're ready to proceed. You can change your mind anytime, even after signing.
- SMP is elective, not medically necessary.It's a tattoo-like procedure, implanting colorants under the skin.
- Results can be difficult to remove.Colorants are natural (carbon, iron oxide).Discomfort or (rarely) pain may occur during treatment.Infection risk exists, though serious cases are rare.
- Consult your doctor if you have skin cancer, sun damage, or other relevant conditions.
- We may require a doctor's note.Follow aftercare instructions and attend follow-ups for best results. Tampering voids our guarantee against fading.
Cancellations, Payments & Complaints:
- We'll notify you of your appointment.
- Confirm attendance to avoid rescheduling.
- Our practitioners are trained, licensed, and insured.48-hour cancellation notice is required.
- Cancellations within 48 hours incur a $250 fee plus full session payment.
- Arrive 15 minutes early. Late arrival may reduce treatment time or forfeit the appointment.We may need to cancel or reschedule; we'll try to notify you in advance but don't offer compensation.
- We'll inform you of pre-treatment preparation.
Deposits and Payments:
- A non-refundable deposit may be required.
- It's deducted from the treatment cost but kept if you cancel or miss your appointment.
- With 48-hour notice, the deposit transfers to the new appointment.
- We accept cash, debit/credit cards (3% fee), or certified bank checks.50% of the total cost is due before the first treatment.
- Quotes are valid for one year.
Treatment Suitability & Liability:
- We assess suitability beforehand and inform you if it's not recommended.
- We're not liable for economic losses or indirect damages related to treatment.
- Disclose all relevant medical information; we're not liable for issues arising from non-disclosure.
- Follow our care instructions.
- We're not liable for death or injury due to negligence.
New Beginnings Micro Guarantee:
- We guarantee against unforeseen fading for 12 months after completing all quoted sessions.
- Style changes (hairline, density, color, etc.) aren't covered and incur fees.
- Laser removal costs are your responsibility.
Acknowledgements:
- I've read and understood this information and documents provided.
- SMP is permanent; changes/removal aren't guaranteed.
- Results vary; some may not respond fully.
- Multiple sessions (minimum 2) and maintenance may be needed.Treatment will be tailored to my needs by a certified practitioner.
- Redness and slight swelling are normal; blistering/crusting is uncommon but possible. I'll contact New Beginnings Micro if it occurs.
- I'll follow care instructions before, during, and after treatment.
- I've disclosed any skin conditions, medical conditions, or medications that may affect treatment. I'll inform New Beginnings Micro if I'm actively tanning.
- New Beginnings Micro isn't liable for issues due to not following aftercare.
- I've had the chance to discuss SMP and ask questions.
- I'm over 18 and have provided proof if needed.My information is accurate.
- Photos may be taken and used by New Beginnings Micro.I consent to receive SMS texts and to the storage/processing of my data (including photos).
